You may pick up your serum refill 10 (ten) business days after submitting this form. Our office will contact you if there are any questions regarding your request.
*Please note, we cannot process sublingual (SLIT) drop orders until payment is received. SLIT orders are ready 10 business days after payment is completed.
For mailing options, serum will not be mailed until mailing fee is received. Serum will be mixed and ready within 10 business days. Failure to make your mailing fee payment will result in the serum being ready to be picked up in the Frisco office on the 10th business day. If you submit a mailing fee after this date, please call the office to request that the serum be mailed. This may result in a delay in receiving your serum.
A charge is billed to your account when allergy vials are mixed. For serum covered by insurance, a claim will be submitted to your insurance company. Patients are responsible for any applicable deductible, coinsurance or copay amounts as per your policy benefits. Patients who fail to pick up serum will still be responsible for the cost of serum.
No appointment is necessary to pick up allergy serum. You may pick up serum during allergy clinic hours.
