What Is INSPIRE Therapy?
INSPIRE is an implantable device used to treat obstructive sleep apnea (OSA). It works inside your body to help keep your airway open while you sleep.
Unlike CPAP, which uses a mask, INSPIRE delivers gentle electrical stimulation to prevent the tongue from blocking the airway.
What Is Obstructive Sleep Apnea?
Obstructive sleep apnea occurs when the airway repeatedly collapses or becomes blocked during sleep, leading to:
- Loud snoring
- Pauses in breathing
- Poor sleep quality
- Daytime fatigue
Who Is a Candidate for INSPIRE?
INSPIRE may be recommended if you:
- Have moderate to severe sleep apnea
- Have tried CPAP but cannot tolerate it
- Are not significantly overweight
- Meet specific airway criteria determined by your ENT doctor
A specialized exam called a sleep endoscopy is typically performed beforehand to confirm candidacy.
How Does INSPIRE Work?
The INSPIRE system consists of three components placed under the skin:
- A small device implanted in the upper chest
- A breathing sensor that monitors your breathing pattern
- A stimulation lead that activates the nerve controlling the tongue
During sleep, when you inhale, the device sends a mild signal that moves the tongue slightly forward, helping keep the airway open.
How Is the Procedure Performed?
- Performed under general anesthesia
- Small incisions are made in the neck and upper chest
- The device and leads are implanted under the skin
- No external components are visible
The procedure typically takes 1-2 hours.
What to Expect After Surgery
- Most patients go home the same day
- Mild soreness or swelling in the chest or neck for a few days
- The device is activated about 3–4 weeks after surgery to allow healing
- The device activation is done at the sleep medicine specialist’s office
- Settings are gradually adjusted for optimal results
- A handheld remote is used to turn the device on at night and off in the morning

Post-Operative Instructions for INSPIRE Procedure
How You Will Feel
You may feel drowsy and tired after surgery due to anesthesia. Most patients rest intermittently the evening after surgery and begin regaining energy the following day. Light activity is encouraged, as it may help speed recovery.
Nausea
Some nausea may occur despite preventive medications. If this happens:
- Rest and limit activity
- Stick to clear liquids
- Avoid heavy foods
Nausea typically resolves within 12–24 hours.
Discomfort
Mild discomfort in the neck and chest is common and usually due to muscle soreness.
- Take prescribed pain medication as directed
- Consider taking a dose the evening of surgery and the following morning
- After 1–2 days, discomfort is often manageable with acetaminophen (Tylenol) and ibuprofen (Motrin)
Swelling and Bruising
- Swelling and bruising of the neck and chest are common and may increase overnight
- Apply ice packs gently for the first 48 hours
- Sleep with your head elevated (2 pillows if possible)
Most swelling improves within 5-7 days.
Fever
- A mild fever (up to 101°F) is common
- Use acetaminophen (Tylenol) or ibuprofen (Motrin) as needed
Antibiotics and Infection
- You received antibiotics during surgery and may be prescribed additional antibiotics
- Infection is rare, but call if you notice:
- Increasing redness
- Swelling with tenderness
- Fever over 102°F that does not respond to medication
Activity
- Resume light activity as tolerated
- Avoid heavy lifting or straining for 1 week
- Avoid strenuous activity or sports for 2 weeks
- Most patients return to work within 5–7 days
Diet
- No specific dietary restrictions
- Gradually return to a normal diet as tolerated
Dressings and Incision Care
- Incisions may be closed with stitches or Dermabond (skin glue)
- Dermabond will peel off naturally in 7–10 days
Personal Hygiene
- Shower or bathe after 24 hours
- Gently clean the incision with soap and water
- Avoid scrubbing the area
Numbness
Temporary numbness around the incision, cheek, or ear is common and typically improves over weeks to months.
Facial Weakness
Temporary mild weakness near the corner of the mouth may occur. This is uncommon and usually resolves within a few weeks. Permanent weakness is rare.
Tongue Weakness
- Temporary tongue weakness or soreness is common
- May affect speech or swallowing
- Typically resolves within 1–2 weeks
Follow-Up
- First visit: 1–2 weeks after surgery for wound check
- Device activation: 3–4 weeks after surgery with the INSPIRE representative and the sleep medicine physician
